If im not mistaken , smart attack = your hero will attack enemy nearest ur a-click ground. Quick attack is u just click a and your hero will attack nearest target.
i use quickcast for skills and for 3 out of 6 itemslots, works pretty fine although id say its a lil bit suboptimal to use it on all the heroes. im too lazy to change it every time tho.
i personally use quickcast on everything except inventory slot 4 which i usually use a tp or force or glimmer cape in. so i can double tap them.
Quickcast is an instant +300 mmr.
the way i have my hotkeys for abilities is quickcast on everything and Shift ability for non quickcast, so if there is a situation i want to check the aoe of a spell or something, Eg. shrapnel, i can hold shift and q and check the aoe of it without casting it.
I have absolutely no idea why people wouldn't use quickcast for abilities (having alt+ for casting them non-quickcast is helpful btw) but seriously, apart from habit i don't see why you would want to never use quickcast. there are one or two spells like brood's webs or sleight of fist that it is nice to see the aoe of but generally on a targetted spell you gain nothing from having to click the target after pressing a key apart from lag.
for items i have a mix of quick cast and non-quickcast bindings because you can't double click selfcast if quickcast is on and it is really embarassing tp'ing 50 units away....
Quickcast is horrible on Io. I would like to be able to have different keybinds for every hero.
Its all about what you are used to. That 1 Click wont make the difference at all. Many setteled Pros like:
Singsing
Burning
Dendi
Sylar
play still with legacy keys and autoattack on, because they are used to it. If you played 1000 of hours on legacy keys, switching to qwer won't step up your game. I used to play with legacy keys, too. Switching took me a few days, and now I am comfortable with. Its not like quickcast brings you 300 mmr instant.

quickcast has nothing to do with legacy, and ALL the pros use it

if u talk about spells, most people put QWER for quickcast and Alt+QWER for normal casting (only 1 extra button)
For items, u have 3-5 slots with quickcast, and the rest goes for normal (no extra button)
Quickcast makes it much easier to not fuck up combos that require tons of clicks. So if you play tinker and nyx you're going to like it. But I've found that even when playing Sven, if I had quickcast on my stun I would've got a kill, instead I had a few ms delay that made me not get a kill after blink + stun attempt. I have quickcast on my inventory slots 2 and 3, and I usually use them for blink/medallion/urn, etc, and reserve other slots for shit like glimmer cape, tp, forcestaff, that I will have to double tap. Quickcast is also really good for legion when you need to catch people in (somewhat) tricky duels when you're forced to chase people.
@i spread love: i think you are right that a few pros do use legacy keys and i've seen a few streams where they don't use quickcast either - i've a feeling bulldog doesn't use quickcast.
it doesn't mean it is best - even if there is only a marginal gain for a pro to change, for a mere mortal i guarantee the difference will be worth it after a few days of muscle memory reprogramming.
there is literally zero benefit from having to click a mouse button after pressing a key rather than just pressing a key.
ps how do people without quickcast orb-walk? is every glaives/frost arrow etc. a two step process?
quickcast is just the next step after not clicking ability icons to cast a spell.
the only reaosn some pros can avoid it is that they are really really good mechanically but they could still shave off a few ms if they switched.
I was storm picker in last patch, so to dodge target stuns i used to play quick cast ulti, then i just got addicted to them but still using qc only on ultimate.
Damn I never used this quickcast thing and now I feel like I've been living a lie... I mean I heard about it but I never thought it was this important.
Should I change my setting or something? Im so used to normal cast, but now I understand how some players use their abilities so perfectly quick.
Have I been playing disadvantaged all this time?

it will take a while to get used to it and u will feel uncomfortable at first, but its definetely worth it
definitely worth it , try the quickcast ability. especially on the storm there is a fucking delay between you press R and click, that delay is reduced with quickcast.
Quick cast all the way.
Although I wouldn't recommend it to newer players until you are familiar with the ranges and AoEs of skills.

I use one key (Alt+D) Quickcast of items. qwer gt, and with alt for quickcast with those buttons
I never used to play with Auto-attack, but last 5 months I've started doing it and finds it pretty useful, saves some attack moves. I hate the fact the hold command doesn't dissable autoattack until you do something tho. I'm pretty sure thats bugged
i use legacy but sometimes i convert my key onto customs when i played rubick (especially when some bugs occured when you stole skills), phx, etc (quickcast heroes)
