To be real, to maintain good consistent performance you actually have to have good health in real life, also playing too much doesn't necessarily make you better, but can actually make you worse. (This is a case of playing way too much.)
Take a break, don't play too much. Stop playing for 2 days, watch your games. Go outside, work out.
You can be consistent by being self concious of everything you do while playing. This way you make sure you'll play the best you can. Keep in mind that consistency on your play doesn't mean you'll always win, you still depend on other four people.
Stay hydrated
Don't play when hungry
Don't play when tired
Have some physical condition, exercising frequently helps (I'm guilty of putting it off)
Take a look at your replays occasionally and look at your mistakes. Take notes in a book/notepad file so you can see recurring themes over time. These are the kinds of things you can work on and significantly improve your game over time.
Don't get too emotionally invested in the game. As upset as you may be flaming allies has about zero positive influence on your games and once they start doing poorly your game gets hard too.
