You aren't a mid player, I have seen how your mids perform and they're not great. You can't be relied on to farm well, I have had a couple games with you where I outfarm/carried you as a support. You rely on low skill items and heroes to win you games...this will keep you low skilled.
Lets go through some of your top heroes... (I will leave out SF cause anyone can play him, success with the hero is harder to gauge)
Puck - Requires positional awareness, and strong decision making as well as the ability to dodge the right targeted spells, how long to hold the jaunt, when/where to blink etc. You don't have that.
Slark - Shouldn't be too hard, a good hero with a mix of farming and fighting in both early and late. Perhaps work on improving this particular hero. I am no expert but maxing dark pact has more benefits overall but may depend on the lineup you're against.
IO - Good idea in theory but need the team to understand how to take advantage of the heroes strengths which I feel won't happen in a high skill game. I could be wrong, but pick this hero sparingly.
Morph - Requires farming intelligence as does Antimage, I am not talking about the ability to last hit, more about the ability to farm, where to find it, how to get it, farming heroes as well as creeps etc.
I think your weaknesses need to be worked on...some of them are.
1. Hero picks
2. Decision making
3. Item choices
4. Flexibility in roles (identify which aren't for you)
